class="elsevierStylePara elsevierViewall">As doenças do foro cardiovascular lideram as taxas de morbi‐mortalidade em Portugal, tornando evidente a importância da doença aterosclerótica no panorama da saúde pública, bem como a necessidade de se implementarem medidas que visem a sua prevenção primária e secundária<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a>.</p><p id="par0070" class="elsevierStylePara elsevierViewall">O estudo <span class="elsevierStyleItalic">Framingham General Cardiovascular Disease risk score</span> (2008) identificou como principais fatores de risco para a doença cardiovascular (DCV): idade, género, colesterol total (mg/dL), colesterol HDL (mg/dL), pressão arterial sistólica (mmHg), diabetes <span class="elsevierStyleItalic">mellitus</span> (DM) e hábitos tabágicos<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>.</p><p id="par0075" class="elsevierStylePara elsevierViewall">De assinalar que, apesar do largo espetro de fatores de risco predisponentes à DCV, uma grande maioria é modificável, oferecendo assim uma janela de atuação para reduzir significativamente o peso da DCV a nível mundial.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Os programas de reabilitação cardíaca (PRC) centrados no exercício ocupam uma posição de destaque na prevenção e tratamento da DCV, bem como no controlo dos seus fatores de risco<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a>.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Uma parte significativa da eficácia dos PRC prende‐se com o seu impacto positivo na capacidade funcional (CF) dos doentes, que se traduz na melhoria do prognóstico e na sobrevida após o diagnóstico da DCV. Atualmente a CF de um doente após evento cardiovascular é reconhecida como um preditor forte e independente de mortalidade<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a>.</p><p id="par0090" class="elsevierStylePara elsevierViewall">A integração em PRC tem demonstrado uma melhoria na tolerância ao esforço, no aumento do limiar isquémico, no controlo mais efetivo dos fatores de risco cardiovascular (FRCV) e no estado geral de saúde<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,7</span></a>.</p><p id="par0095" class="elsevierStylePara elsevierViewall">O presente estudo tem como objetivo primário determinar quais os FRCV que podem atuar como preditores da evolução da CF em doentes que completam um PRC.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Material e métodos</span><p id="par0100" class="elsevierStylePara elsevierViewall">Realizou‐se um estudo descritivo transversal retrospetivo que incluiu os doentes com diagnóstico de doença cardíaca isquémica referenciados consecutivamente, entre janeiro de 2008 e dezembro de 2013, para um PRC de carácter multidisciplinar, com base no exercício.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Programa de reabilitação cardíaca</span><p id="par0105" class="elsevierStylePara elsevierViewall">O PRC incluiu aconselhamento individual sobre estratégias para controlo dos FRCV, sessões de educação para a saúde em grupo e sessões de exercício físico supervisionado.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Exercício físico supervisionado</span><p id="par0110" class="elsevierStylePara elsevierViewall">Todos os doentes integraram um programa de exercício físico supervisionado por médicos (fisiatra e cardiologista) e apoio técnico de fisioterapeuta, durante 8‐12 semanas, com periodicidade bissemanal. Cada sessão, com duração entre 60‐90 minutos, englobou um protocolo de exercício constituído por: fase de aquecimento, treino aeróbico (exercício em tapete rolante, cicloergómetro de membros superiores e inferiores), treino de força (com recurso a bandas elásticas, halteres, bolas medicinais e aparelhos de fortalecimento muscular) e fase de relaxamento/exercícios de flexibilidade. A intensidade do exercício aeróbico foi determinada individualmente, com base no cálculo de frequência cardíaca de treino através do método de Karvonen, utilizando os dados recolhidos na prova de esforço e complementada com a perceção subjetiva do esforço aferida na escala de Borg<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Adicionalmente, foi incentivada a prática de exercício físico nos restantes dias da semana, de encontro com as recomendações no contexto da prevenção secundária, visando o gasto calórico semanal adequado (<a class="elsevierStyleCrossRef" href="#tbl0005">Tabela 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0120" class="elsevierStylePara elsevierViewall">A análise foi elaborada com programa <span class="elsevierStyleItalic">Statistical Package for the Social Sciences</span> (SPSS) 21.0. As variáveis categóricas foram descritas através das frequências absolutas e as relativas em percentagem, enquanto as variáveis contínuas foram descritas utilizando a média e desvio padrão, segundo uma regressão linear univariada e multivariada para as variáveis selecionadas. Considerou‐se como variável dependente a evolução da CF estimada em equivalentes metabólicos (MET) obtidos na prova de esforço, realizada no início e no final do programa. As variáveis independentes consideradas foram: idade, género, dislipidemia, DM, hábitos tabágicos, índice de massa corporal (IMC), nível de atividade física (avaliado através do questionário <span class="elsevierStyleItalic">International Physical Activity Questionnaire</span> [IPAQ]) e diagnóstico de admissão a PRC. Este questionário foi validado para a população portuguesa e visa quantificar (MET*min*sem) a atividade física realizada ao longo de uma semana, nos diversos contextos das atividades diárias. Permite caracterizar de acordo com os limites: ‹<span class="elsevierStyleHsp" style=""></span>600<span class="elsevierStyleHsp" style=""></span>MET*min*sem para nível de atividade física baixa (sedentarismo); 600‐3000<span class="elsevierStyleHsp" style=""></span>MET*min*sem correspondente a nível de atividade física moderado; superior a 3000<span class="elsevierStyleHsp" style=""></span>MET*min*sem para níveis de atividade física elevados<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a>.</p></span></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Resultados</span><p id="par0125" class="elsevierStylePara elsevierViewall">Este estudo envolveu 1399 doentes. A média de idades foi de 61 anos (desvio padrão de:<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>11), verificando‐se que 1068 (76%) doentes eram do sexo masculino. Relativamente aos FRCV analisados neste estudo a prevalência de dislipidemia foi de 71%, DM 38% e hipertensão arterial (HTA) 61%. O IMC apresentava‐se abaixo dos 25<span class="elsevierStyleHsp" style=""></span>Kg/m<span class="elsevierStyleSup">2</span> (baixo peso ou peso normal) em 33%, 49% apresentavam excesso ponderal (IMC 25‐30) e 18% obesidade (IMC ≥<span class="elsevierStyleHsp" style=""></span>30). Observou‐se a presença de hábitos tabágicos em 31% da amostra (<a class="elsevierStyleCrossRef" href="#tbl0010">Tabela 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0130" class="elsevierStylePara elsevierViewall">De acordo com o resultado do IPAQ, a grande maioria da amostra (63%) apresentava baixo índice de atividade física (sedentarismo), previamente ao programa, e apenas 3% realizava atividade vigorosa. De destacar que nos últimos dois anos se registou um maior índice de sedentarismo com valores de 72 e 76% em 2012 e 2013, respetivamente (<a class="elsevierStyleCrossRef" href="#tbl0015">Tabela 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0135" class="elsevierStylePara elsevierViewall">O diagnóstico de admissão mais frequente foi síndrome coronária aguda (SCA) (62%) (<a class="elsevierStyleCrossRef" href="#tbl0020">Tabela 4</a>).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0140" class="elsevierStylePara elsevierViewall">Da amostra inicial de 1399, apenas se obteve a totalidade dos dados no final do programa em 1125 doentes, sendo os principais motivos desta diminuição da amostra a perda de dados no <span class="elsevierStyleItalic">follow‐up</span> ou abandono do PRC (motivos clínicos ou pessoais).</p><p id="par0145" class="elsevierStylePara elsevierViewall">A perda da amostra registada, não apresentou alterações significativas na prevalência dos diferentes fatores de risco (<a class="elsevierStyleCrossRef" href="#tbl0025">Tabela 5</a>).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0155" class="elsevierStylePara elsevierViewall">Assim, a análise relativa à influência dos FRCV na evolução da CF abrangeu unicamente esta amostra populacional.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Verificou‐se uma evolução média da CF na fase <span class="elsevierStyleSmallCaps">II</span> de 1,45<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1,19 MET. A máxima evolução registou um valor de 7,8<span class="elsevierStyleHsp" style=""></span>MET (<a class="elsevierStyleCrossRef" href="#tbl0030">Tabela 6</a>). Constatou‐se ainda que 93% dos doentes aumentaram a sua CF (<a class="elsevierStyleCrossRef" href="#tbl0035">Tabela 7</a>).</p><elsevierMultimedia ident="tbl0030"></elsevierMultimedia><elsevierMultimedia ident="tbl0035"></elsevierMultimedia><p id="par0165" class="elsevierStylePara elsevierViewall">Utilizando um modelo de regressão linear multivariado para estabelecer a influência dos diferentes fatores de risco sobre a evolução da CF na prova de esforço, obteve‐se um ajustamento baixo (R<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,213,R<span class="elsevierStyleSup">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,045). No entanto, observou‐se que os doentes com DM tiveram uma pior evolução da CF relativamente aos que não tinham DM, sendo que esta evidência foi estatisticamente significativa.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Outro dos fatores de risco associados a alterações na evolução da CF foi a idade. Constatou‐se que doentes entre os 45 e os 65 anos e doentes com mais de 65 anos tiveram uma maior evolução da CF do que os doentes com menos de 45 anos. É ainda de destacar que os doentes admitidos para PRC após cirurgia de revascularização coronária (CABG), obtiveram em média uma evolução da CF superior (0,44<span class="elsevierStyleHsp" style=""></span>MET) relativamente aos que foram admitidos na unidade com SCA, com significado estatístico. Pelo contrário doentes pós‐angioplastia eletiva e doentes que entraram no programa para controlo dos FRCV obtiveram uma menor evolução da CF, quando comparados com os doentes admitidos por SCA.</p><p id="par0175" class="elsevierStylePara elsevierViewall">Todas as outras variáveis incluídas no estudo não obtiveram significado estatístico (<a class="elsevierStyleCrossRef" href="#tbl0040">Tabela 8</a>).</p><elsevierMultimedia ident="tbl0040"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Discussão</span><p id="par0180" class="elsevierStylePara elsevierViewall">Este estudo aponta para a existência de três fatores (quando analisados multivariadamente) que constituem preditores da evolução da CF nos PRC, sendo eles a idade, a presença de DM e o diagnóstico de admissão.</p><p id="par0185" class="elsevierStylePara elsevierViewall">Estabeleceu‐se uma relação estatisticamente significativa entre a presença de DM e uma pior evolução da CF ao longo do PRC. Há evidências na literatura médica que doentes com DM possuem uma disfunção ao nível do músculo‐esquelético associada à doença microvascular, condicionando uma baixa CF. Esta disfunção corresponde a uma maior perda de fosfocreatina, um maior decréscimo do pH e uma mais rápida desoxigenação do músculo‐esquelético durante o exercício, associando‐se também uma menor recuperação pós exercício<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,11</span></a>. A juntar‐se a estes dados temos também evidências que apontam para uma maior exigência de oxigénio ao nível do miocárdio dos doentes com DM, comparativamente aos não diabéticos<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a>. O aumento da disponibilidade de ácidos gordos (presente nos diabéticos) resulta num aumento da absorção e oxidação na mitocôndria do músculo cardíaco e aumento da expressão mitocondrial de proteínas desacopladoras, ambos conduzindo a uma diminuição da quantidade de ATP produzido por molécula de oxigénio consumida<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12,13</span></a>.</p><p id="par0285" class="elsevierStylePara elsevierViewall">Face a estes dados, podemos antever que doentes diabéticos vão ter, à partida, uma pior evolução, pelo que poderá ser vantajoso um maior foco nestes doentes, instituindo‐se programas com maior frequência semanal, ou tentando assegurar uma maior adesão à realização de exercício fora das sessões pré‐estabelecidas.</p><p id="par0190" class="elsevierStylePara elsevierViewall">A idade demonstrou igualmente ser um fator preditor, estatisticamente significativo, da evolução da CF. Neste estudo verificou‐se que indivíduos de meia‐idade (45‐65 anos) e indivíduos mais idosos (mais de 65 anos) obtiveram uma melhor evolução da CF do que indivíduos mais jovens (menos de 45 anos). Estes achados são concordantes com outros apresentados por estudos semelhantes, que relataram ganhos relativos em CF superiores nos doentes mais idosos<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14,15</span></a>. A CF declina cerca de 8‐10% por década nos indivíduos não atléticos, sendo isto mediado em grande parte pela diminuição da frequência cardíaca máxima e da capacidade de oxigenação arteriovenosa<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a>. Contudo, quando submetidos a programa individualizado e adaptado, estes doentes apresentam os mesmos benefícios que os restantes doentes e com ganhos de CF até superiores, o que poderá estar relacionado com o facto de apresentarem também uma CF basal inferior.</p><p id="par0195" class="elsevierStylePara elsevierViewall">Estes dados vão no sentido de uma maior integração dos doentes mais idosos nestes programas, visto ser claro o benefício funcional que estes apresentam.</p><p id="par0200" class="elsevierStylePara elsevierViewall">O excesso de peso e a obesidade não revelaram ser elementos preditores da evolução da CF com significado estatístico. Esta observação é discordante de outros estudos, em que a obesidade foi associada a uma pior performance em cada sessão de treino na fase <span class="elsevierStyleSmallCaps">II</span> do PRC, levando a menores ganhos obtidos ao nível da CF no final do programa<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a>. Foi também descrita a relação inversa entre obesidade e a CF<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a>. De facto, estudos mostraram que a deposição excessiva de massa adiposa corporal leva a uma menor captação de oxigénio pelos músculos esqueléticos e consequentemente a um menor desempenho funcional<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a>. Para além disso, nos indivíduos obesos existe um aumento nas fibras musculares tipo <span class="elsevierStyleSmallCaps">II</span> e uma diminuição nas fibras musculares tipo <span class="elsevierStyleSmallCaps">I,</span> que também está associado a uma importante redução na captação do oxigénio<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a>. Deve‐se, deste modo, destacar que o facto dos doentes obesos não terem tido a pior performance esperada pode ser explicado pelas estratégias individualizadas a que estes foram submetidos durante todo o programa, relativamente aos parâmetros do exercício e ao plano alimentar.</p><p id="par0205" class="elsevierStylePara elsevierViewall">Ao analisarmos a evolução dos doentes admitidos no programa após CABG, verificamos que estes evidenciaram uma melhor evolução na CF quando comparados a doentes admitidos por SCA, mas sem intervenção cirúrgica. Este dado pode ser explicado por uma pior CF basal dos doentes pós‐CABG levando a que a evolução da mesma seja mais significativa.</p><p id="par0210" class="elsevierStylePara elsevierViewall">Na questão da influência do género na evolução da CF, não foram observados dados com significado estatístico que apontem neste sentido, o que vai de encontro a observações feitas em estudos semelhantes nesta área<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a>.</p><p id="par0215" class="elsevierStylePara elsevierViewall">A HTA não demonstrou ter influência na CF. Há evidências na literatura médica de uma relação inversa entre a CF dos doentes e a presença de HTA<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">22,23</span></a>. Seria previsível que doentes hipertensos tivessem à partida uma pior CF inicial, mas que ao longo do PRC, com a prática regular de exercício, houvesse possibilidade para uma maior margem de evolução. Apesar de este facto não ter sido observado no nosso estudo é necessário ter em conta que todos os doentes seguidos em PRC estão estritamente vigiados relativamente aos seus fatores de risco e à medicação que efetuam. Assim, a influência da hipertensão sobre a CF pode não ter sido significativa na nossa amostra, pelo controlo dos valores tensionais e otimização da farmacoterapia, aliados ao controlo alimentar com ênfase particular na restrição da ingestão salina.</p><p id="par0220" class="elsevierStylePara elsevierViewall">A dislipidemia também não apresentou influência na evolução da CF. Vários estudos demonstraram uma forte correlação entre a melhoria da CF e a redução no colesterol total, nos triglicerídeos totais e nas lipoproteínas de baixa densidade (LDL), associando‐se igualmente a um aumento significativo nas lipoproteínas de alta densidade (HDL)<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">24,25</span></a>.</p><p id="par0225" class="elsevierStylePara elsevierViewall">A identificação dos hábitos tabágicos dos doentes no momento em que iniciaram o programa não revelou ter influência na evolução da CF. Sabe‐se que o tabagismo está inversamente relacionado com este parâmetro. No entanto, está também demonstrado que esta disfunção pode ser revertida, pelo menos parcialmente, com a cessação tabágica sendo esses efeitos evidentes a longo prazo. Provavelmente se os dados de <span class="elsevierStyleItalic">follow‐up</span> forem avaliados a longo prazo, seria encontrada uma relação favorável entre a cessação tabágica e a recuperação da CF<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">26,27</span></a>.</p><p id="par0230" class="elsevierStylePara elsevierViewall">O sedentarismo (avaliado pelo IPAQ) também não revelou ser um fator preditor da evolução da CF dos doentes. O comportamento sedentário está associado a alterações periféricas que levam a uma diminuição da CF, que incluem perda progressiva de músculo‐esquelético e disfunção na capacidade oxidativa do músculo<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">28,29</span></a>. Há também evidências que apontam para uma relação entre o sedentarismo e uma disfunção vascular, possivelmente associada a uma diminuição da vasodilatação endotélio‐dependente e um maior dano celular endotelial<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">30,31</span></a>. A baixa influência do nível de atividade física na evolução da CF, registada entre os diferentes grupos, poderá ser explicada pelo facto de, nos doentes que já praticavam níveis moderados a vigorosos de AF, existir uma menor evolução, devido a uma maior adaptação física prévia ao programa. Nos doentes sedentários, todos os fatores referidos anteriormente atuam em conjunto, limitando os resultados obtidos na fase <span class="elsevierStyleSmallCaps">II</span> do PRC.</p><p id="par0235" class="elsevierStylePara elsevierViewall">É necessário destacar que o número de doentes sedentários a entrar no PRC, aumentou ao longo dos anos, sendo que em 2013 apresentou o seu valor máximo atingindo os 76%. Este facto é preocupante, tendo em conta diversos estudos que estabelecem uma relação significativa entre o baixo nível de atividade física e o maior risco de eventos cardiovasculares<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">32</span></a>. De facto, a <span class="elsevierStyleItalic">World Health Organization</span> estabeleceu o sedentarismo como sendo o 4.° principal fator de risco de mortalidade a nível mundial<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">33</span></a>.</p><p id="par0240" class="elsevierStylePara elsevierViewall">Finalmente destacar que independentemente dos FRCV apresentados pelos doentes e da prevalência destes na amostra, houve um aumento da CF no final da fase <span class="elsevierStyleSmallCaps">II</span> em 93% dos 1125 doentes observados. Isto é um dado que vem reforçar ainda mais a importância deste tipo de programas pela sua implicação no prognóstico vital destes doentes.</p><p id="par0245" class="elsevierStylePara elsevierViewall">Este estudo apresenta algumas limitações que podem influenciar os resultados obtidos. O facto de terem sido considerados apenas os fatores de risco com que os doentes iniciaram o programa e não ter sido analisada a evolução desses fatores ao longo do programa, pode condicionar a correlação entre esses fatores e a evolução da CF.</p><p id="par0250" class="elsevierStylePara elsevierViewall">Fatores psicológicos e sociais não foram avaliados neste estudo, o que de certa forma limita as conclusões a retirar do mesmo, visto estar evidenciada a relação destes fatores com a adesão e a performance dos doentes durante os PRC<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">34</span></a>. A não integração da medicação cumprida pelos doentes, bem como os dados relativos à gravidade da DAC (número de vasos atingido, revascularização, presença de disfunção sistólica e/ou diastólica) retirou variáveis que podem igualmente condicionar os resultados obtidos.</p><p id="par0255" class="elsevierStylePara elsevierViewall">Atendendo ao grande impacto que a CF tem na sobrevida destes doentes, preconiza‐se que se estabeleçam os diferentes fatores preditores, não só relativos aos FRCV, mas também relacionados com fatores demográfico‐sociais e psicológicos (que não foram analisados neste estudo) de forma a criar um programa que possibilite a máxima evolução dos doentes. Não podemos, no entanto, assumir que a resposta funcional dos doentes depende exclusivamente dos fatores endógenos. De facto, os resultados obtidos neste estudo demonstram que os fatores analisados tiveram pouca influência na evolução dos doentes, o que leva a colocar a hipótese que a resposta aos PRC é multifatorial.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Conclusões</span><p id="par0260" class="elsevierStylePara elsevierViewall">Este estudo reforça a relevância dos PRC na prevenção secundária da DCV, demonstrando que a grande maioria dos doentes que neles participa apresenta uma evolução significativamente favorável da CF, identificando subgrupos de doentes que apresentam uma recuperação menos evidente, o que aponta para a necessidade de estratégias de atuação individualizadas.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Responsabilidades éticas</span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Proteção de pessoas e animais</span><p id="par0265" class="elsevierStylePara elsevierViewall">Os autores declaram que para esta investigação não se realizaram experiências em seres humanos e/ou animais.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Confidencialidade dos dados</span><p id="par0270" class="elsevierStylePara elsevierViewall">Os autores declaram que não aparecem dados de pacientes neste artigo.</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Direito à privacidade e consentimento escrito</span><p id="par0275" class="elsevierStylePara elsevierViewall">Os autores declaram ter recebido consentimento escrito dos pacientes e/ou sujeitos mencionados no artigo. A variável dependente é a evolução da CF estimada em equivalentes metabólicos (MET) obtidos na prova de esforço, realizada no início e no final da fase <span class="elsevierStyleSmallCaps">II</span> do programa. As variáveis independentes foram a idade, género, dislipidemia, diabetes <span class="elsevierStyleItalic">mellitus</span>, hábitos tabágicos, índice de massa corporal, nível de atividade física e diagnóstico de admissão a PRC.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Resultados</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">A amostra incluiu 1399 doentes dos quais 1125 (80,4%) finalizaram a fase <span class="elsevierStyleSmallCaps">II</span> do programa. Verificou‐se melhoria da CF na maioria dos doentes (93%) com um ganho médio de 1,45<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1,19<span class="elsevierStyleHsp" style=""></span>MET. Os doentes na faixa etária (45‐65] e superior a 65 anos obtiveram um incremento na CF superior quando comparados com a restante faixa etária. Os doentes admitidos para PRC após <span class="elsevierStyleItalic">Coronary Artery Bypass Graft</span> obtiveram um benefício superior da CF quando comparados com doentes com síndrome coronária aguda. Os doentes não diabéticos obtiveram um incremento da sua CF superior comparativamente com doentes diabéticos. The dependent variable was changes in FC estimated in metabolic equivalents (METs) achieved in stress testing at the beginning and end of the phase II program. The independent variables were age, gender, dyslipidemia, diabetes, smoking, body mass index, physical activity level and reason for referral to the CRP.</p></span> <span id="abst0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Results</span><p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">The sample included 1399 patients, of whom 1125 (80.4%) completed the program. FC improved in most patients (93%), with a mean gain of 1.45±1.19 METs. Patients aged 45 ‐65 and over 65 years achieved a greater increase in FC compared with other age groups. Patients admitted to the CRP after coronary artery bypass graft surgery obtained a greater improvement in FC compared to patients with acute coronary syndrome. Non‐diabetic patients benefited more than diabetic patients.
