We appreciate the interest shown by M Kalçık, MO Gürsoy, S Gündüz and M Özkan in our article entitled “Prosthetic mitral valve thrombosis in pregnancy: from thrombolysis to anticoagulation”.1 The comments in their letter to the Editor are in line with our discussion of this case and with our brief review of prosthetic valve thrombosis (PVT), from prevention (with anticoagulant therapy) to treatment by fibrinolysis.
In the case described, the PVT occurred before 2003, as is implied in the text. At that time there was no evidence in the form of controlled trials comparing different types of fibrinolysis for treatment of PVT, and streptokinase was the first-line thrombolytic in most centers. Indeed, it was used in 2000 as the first-line agent by most of the authors who now comment on our article.2
