episodes predict poor clinical outcome regardless of symptoms&#46; Potential benefits of remote monitoring are early arrhythmia detection and patient continuous monitoring&#46; Several studies of device remote monitoring consistently demonstrated that AF represents the most common clinical alert and that detailed information on arrhythmia onset&#44; duration&#44; and burden as well as on the ventricular rate may be early available for clinical evaluation&#46; Reaction time to AF alerts was very short in all series involving either pacemakers or defibrillators and action ability of AF alerts was very high&#46; In the Home Guide Registry&#44; in which 1650 patients were enrolled&#44; AF was detected in 16&#46;3&#37; of patients and represented 36&#37; of all cardiovascular events during the follow-up&#46; Timely anticoagulation introduction in asymptomatic patients may impact on the stroke rate&#46; According to the results of repeated Monte Carlo simulations based on a real population of 166 patients&#44; daily monitoring may reduce the 2-year stroke risk by 9-18&#37; with an absolute reduction of 0&#46;2-0&#46;6&#37;&#44; compared with conventional inter-visit intervals of 6-12 months&#46; In the COMPAS trial&#44; the incidence of hospitalizations for atrial arrhythmias and related stroke was significantly higher in the control group than in the remote monitoring group&#46; Major questions will be addressed by the ongoing IMPACT trial in which a remote monitoring guided anticoagulation strategy based on AF detection will be compared with a physician-directed standard strategy&#46; In patients with heart failure&#44; AF early detection combined with other indexes may help prevent hospitalizations&#46;</p></span><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0010">Coment&#225;rio</span><p id="par0020" class="elsevierStylePara elsevierViewall">Neste interessante artigo de Ricci<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>&#44; este autor faz uma revis&#227;o das mais recentes evid&#234;ncias que suportam a utilidade da dete&#231;&#227;o de epis&#243;dios assintom&#225;ticos de fibrilha&#231;&#227;o auricular &#40;FA&#41; atrav&#233;s da monitoriza&#231;&#227;o &#224; dist&#226;ncia dos doentes portadores de <span class="elsevierStyleItalic">pacemakers</span> definitivos &#40;PMD&#41; e cardioversores&#47;desfibrilhadores &#40;CDI&#41;&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">Est&#225; hoje bem documentado que o risco de tromboembolismo secund&#225;rio a FA &#40;embolismo cerebral e perif&#233;rico&#41; pode ser significativo tanto em doentes com FA permanente como em doentes com FA persistente ou mesmo parox&#237;stica&#46; Estes dados resultam de estudos epidemiol&#243;gicos em doentes portadores de dispositivos implantados<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> ou que foram monitorizados atrav&#233;s de exames de Holter<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>&#46;</p><p id="par0030" class="elsevierStylePara elsevierViewall">O risco emb&#243;lico &#233; calculado com base em <span class="elsevierStyleItalic">scores</span> de risco &#40;sendo o <span class="elsevierStyleItalic">score</span> CHA<span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">2</span>Vasc o recomendado nas orienta&#231;&#245;es europeias&#41; que englobam v&#225;rias vari&#225;veis cl&#237;nicas&#44; mas com igual peso nos v&#225;rios tipos de FA acima indicados<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a>&#46;</p><p id="par0035" class="elsevierStylePara elsevierViewall">&#201; hoje consensual que o diagn&#243;stico da FA antes do primeiro evento emb&#243;lico &#233; uma prioridade na preven&#231;&#227;o dos acidentes emb&#243;licos cerebrais<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a>&#46; Torna-se assim muito relevante a documenta&#231;&#227;o de epis&#243;dios assintom&#225;ticos de FA que conduzam a uma mais precoce institui&#231;&#227;o de terap&#234;utica anticoagulante nos doentes com <span class="elsevierStyleItalic">scores</span> de risco significativos &#40;ex&#58; CHA<span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">2</span>Vasc<span class="elsevierStyleHsp" style=""></span>&#8805;<span class="elsevierStyleHsp" style=""></span>1&#41;&#46;</p><p id="par0040" class="elsevierStylePara elsevierViewall">A dete&#231;&#227;o destes epis&#243;dios de FA assintom&#225;tica torna-se pouco prov&#225;vel se o seguimento dos doentes se limitar a 2-3 consultas&#47;ano nos casos de FA parox&#237;stica&#46; Mesmo com o recurso a monitoriza&#231;&#245;es do ECG atrav&#233;s da realiza&#231;&#227;o de exames de Holter mais ou menos regulares ou de sistemas externos de dete&#231;&#227;o de eventos&#44; a probabilidade de serem detetados os epis&#243;dios de FA pode ser muito reduzida&#46;</p><p id="par0045" class="elsevierStylePara elsevierViewall">Por outro lado&#44; as indica&#231;&#245;es para coloca&#231;&#227;o de sistemas de dete&#231;&#227;o de eventos disr&#237;tmicos implantados sob a pele s&#227;o ainda relativamente reduzidas e o seu custo elevado&#46;</p><p id="par0050" class="elsevierStylePara elsevierViewall">A possibilidade de analisar os registos obtidos pelos dispositivos implantados por outras indica&#231;&#245;es &#40;PMD e CDI&#41; veio permitir uma maior probabilidade de dete&#231;&#227;o dos epis&#243;dios de FA parox&#237;stica&#46; No entanto&#44; o facto de esta an&#225;lise ser&#44; tradicionalmente&#44; efetuada apenas nas consultas &#171;de <span class="elsevierStyleItalic">pacing</span>&#187;&#44; frequentemente apenas semestrais&#44; faz com que o tempo m&#233;dio &#171;de resposta&#187; entre a ocorr&#234;ncia dos epis&#243;dios de FA e a institui&#231;&#227;o da terap&#234;utica anticoagulante seja muito elevado com consequentes riscos para o doente&#46;</p><p id="par0055" class="elsevierStylePara elsevierViewall">O desenvolvimento de tecnologias de transmiss&#227;o &#224; dist&#226;ncia &#40;a partir do domic&#237;lio dos doentes&#41; dos registos da telemetria dos dispositivos implantados para os centros cardiol&#243;gicos especializados&#44; permite uma dete&#231;&#227;o mais r&#225;pida dos epis&#243;dios de FA e&#44; consequentemente&#44; um tempo de resposta substancialmente mais curto como ficou bem demonstrado nos v&#225;rios estudos&#44; mencionados neste artigo de Ricci&#46;</p><p id="par0060" class="elsevierStylePara elsevierViewall">O recurso a estas tecnologias de monitoriza&#231;&#227;o remota&#44; apesar de &#250;til como aqui demonstrado&#44; est&#225;&#44; no entanto&#44; limitado aos doentes portadores de dispositivos que disponham desta tecnologia&#44; deixando de fora todos os restantes doentes com FA parox&#237;stica n&#227;o portadores destes dispositivos&#46;</p><p id="par0065" class="elsevierStylePara elsevierViewall">A recente dissemina&#231;&#227;o de dispositivos de registo e an&#225;lise de curtas tiras de ritmo &#40;atrav&#233;s&#44; por exemplo&#44; de <span class="elsevierStyleItalic">hardware</span> e aplicativos de <span class="elsevierStyleItalic">software</span> dos mais recentes <span class="elsevierStyleItalic">smartphones</span>&#41; veio permitir o registo de epis&#243;dios de FA parox&#237;stica num crescente n&#250;mero de doentes&#46;</p><p id="par0070" class="elsevierStylePara elsevierViewall">Alguns destes sistemas de an&#225;lise de tiras de ECG foram j&#225; rigorosamente testados<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> o que levou &#224; sua aprova&#231;&#227;o pelas entidades regulamentares apropriadas &#40;p&#46; ex&#46;&#58; do sistema AliveCor<span class="elsevierStyleSup">&#174;</span> que foi j&#225; aprovado pela <span class="elsevierStyleItalic">Food and Drug Administration</span> nos EUA&#41;&#46;</p><p id="par0075" class="elsevierStylePara elsevierViewall">Sendo expect&#225;vel que v&#225;rios outros sistemas semelhantes venham a ser aprovados num futuro pr&#243;ximo&#44; os servi&#231;os de cardiologia ter&#227;o de se adaptar a esta nova realidade&#44; uma vez que o afluxo de dados a analisar ser&#225; substancialmente maior&#46; Este facto levar&#225; a que um maior n&#250;mero de doentes possa ser precocemente diagnosticado e tratado com f&#225;rmacos anticoagulantes&#44; episodes predict poor clinical outcome regardless of symptoms&#46; Potential benefits of remote monitoring are early arrhythmia detection and patient continuous monitoring&#46; Several studies of device remote monitoring consistently demonstrated that AF represents the most common clinical alert and that detailed information on arrhythmia onset&#44; duration&#44; and burden as well as on the ventricular rate may be early available for clinical evaluation&#46; Reaction time to AF alerts was very short in all series involving either pacemakers or defibrillators and action ability of AF alerts was very high&#46; In the Home Guide Registry&#44; in which 1650 patients were enrolled&#44; AF was detected in 16&#46;3&#37; of patients and represented 36&#37; of all cardiovascular events during the follow-up&#46; Timely anticoagulation introduction in asymptomatic patients may impact on the stroke rate&#46; According to the results of repeated Monte Carlo simulations based on a real population of 166 patients&#44; daily monitoring may reduce the 2-year stroke risk by 9-18&#37; with an absolute reduction of 0&#46;2-0&#46;6&#37;&#44; compared with conventional inter-visit intervals of 6-12 months&#46; In the COMPAS trial&#44; the incidence of hospitalizations for atrial arrhythmias and related stroke was significantly higher in the control group than in the remote monitoring group&#46; Major questions will be addressed by the ongoing IMPACT trial in which a remote monitoring guided anticoagulation strategy based on AF detection will be compared with a physician-directed standard strategy&#46; In patients with heart failure&#44; AF early detection combined with other indexes may help prevent hospitalizations&#46;</p></span><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0010">Coment&#225;rio</span><p id="par0020" class="elsevierStylePara elsevierViewall">Neste interessante artigo de Ricci<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>&#44; este autor faz uma revis&#227;o das mais recentes evid&#234;ncias que suportam a utilidade da dete&#231;&#227;o de epis&#243;dios assintom&#225;ticos de fibrilha&#231;&#227;o auricular &#40;FA&#41; atrav&#233;s da monitoriza&#231;&#227;o &#224; dist&#226;ncia dos doentes portadores de <span class="elsevierStyleItalic">pacemakers</span> definitivos &#40;PMD&#41; e cardioversores&#47;desfibrilhadores &#40;CDI&#41;&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">Est&#225; hoje bem documentado que o risco de tromboembolismo secund&#225;rio a FA &#40;embolismo cerebral e perif&#233;rico&#41; pode ser significativo tanto em doentes com FA permanente como em doentes com FA persistente ou mesmo parox&#237;stica&#46; Estes dados resultam de estudos epidemiol&#243;gicos em doentes portadores de dispositivos implantados<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> ou que foram monitorizados atrav&#233;s de exames de Holter<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>&#46;</p><p id="par0030" class="elsevierStylePara elsevierViewall">O risco emb&#243;lico &#233; calculado com base em <span class="elsevierStyleItalic">scores</span> de risco &#40;sendo o <span class="elsevierStyleItalic">score</span> CHA<span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">2</span>Vasc o recomendado nas orienta&#231;&#245;es europeias&#41; que englobam v&#225;rias vari&#225;veis cl&#237;nicas&#44; mas com igual peso nos v&#225;rios tipos de FA acima indicados<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a>&#46;</p><p id="par0035" class="elsevierStylePara elsevierViewall">&#201; hoje consensual que o diagn&#243;stico da FA antes do primeiro evento emb&#243;lico &#233; uma prioridade na preven&#231;&#227;o dos acidentes emb&#243;licos cerebrais<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a>&#46; Torna-se assim muito relevante a documenta&#231;&#227;o de epis&#243;dios assintom&#225;ticos de FA que conduzam a uma mais precoce institui&#231;&#227;o de terap&#234;utica anticoagulante nos doentes com <span class="elsevierStyleItalic">scores</span> de risco significativos &#40;ex&#58; CHA<span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">2</span>Vasc<span class="elsevierStyleHsp" style=""></span>&#8805;<span class="elsevierStyleHsp" style=""></span>1&#41;&#46;</p><p id="par0040" class="elsevierStylePara elsevierViewall">A dete&#231;&#227;o destes epis&#243;dios de FA assintom&#225;tica torna-se pouco prov&#225;vel se o seguimento dos doentes se limitar a 2-3 consultas&#47;ano nos casos de FA parox&#237;stica&#46; Mesmo com o recurso a monitoriza&#231;&#245;es do ECG atrav&#233;s da realiza&#231;&#227;o de exames de Holter mais ou menos regulares ou de sistemas externos de dete&#231;&#227;o de eventos&#44; a probabilidade de serem detetados os epis&#243;dios de FA pode ser muito reduzida&#46;</p><p id="par0045" class="elsevierStylePara elsevierViewall">Por outro lado&#44; as indica&#231;&#245;es para coloca&#231;&#227;o de sistemas de dete&#231;&#227;o de eventos disr&#237;tmicos implantados sob a pele s&#227;o ainda relativamente reduzidas e o seu custo elevado&#46;</p><p id="par0050" class="elsevierStylePara elsevierViewall">A possibilidade de analisar os registos obtidos pelos dispositivos implantados por outras indica&#231;&#245;es &#40;PMD e CDI&#41; veio permitir uma maior probabilidade de dete&#231;&#227;o dos epis&#243;dios de FA parox&#237;stica&#46; No entanto&#44; o facto de esta an&#225;lise ser&#44; tradicionalmente&#44; efetuada apenas nas consultas &#171;de <span class="elsevierStyleItalic">pacing</span>&#187;&#44; frequentemente apenas semestrais&#44; faz com que o tempo m&#233;dio &#171;de resposta&#187; entre a ocorr&#234;ncia dos epis&#243;dios de FA e a institui&#231;&#227;o da terap&#234;utica anticoagulante seja muito elevado com consequentes riscos para o doente&#46;</p><p id="par0055" class="elsevierStylePara elsevierViewall">O desenvolvimento de tecnologias de transmiss&#227;o &#224; dist&#226;ncia &#40;a partir do domic&#237;lio dos doentes&#41; dos registos da telemetria dos dispositivos implantados para os centros cardiol&#243;gicos especializados&#44; permite uma dete&#231;&#227;o mais r&#225;pida dos epis&#243;dios de FA e&#44; consequentemente&#44; um tempo de resposta substancialmente mais curto como ficou bem demonstrado nos v&#225;rios estudos&#44; mencionados neste artigo de Ricci&#46;</p><p id="par0060" class="elsevierStylePara elsevierViewall">O recurso a estas tecnologias de monitoriza&#231;&#227;o remota&#44; apesar de &#250;til como aqui demonstrado&#44; est&#225;&#44; no entanto&#44; limitado aos doentes portadores de dispositivos que disponham desta tecnologia&#44; deixando de fora todos os restantes doentes com FA parox&#237;stica n&#227;o portadores destes dispositivos&#46;</p><p id="par0065" class="elsevierStylePara elsevierViewall">A recente dissemina&#231;&#227;o de dispositivos de registo e an&#225;lise de curtas tiras de ritmo &#40;atrav&#233;s&#44; por exemplo&#44; de <span class="elsevierStyleItalic">hardware</span> e aplicativos de <span class="elsevierStyleItalic">software</span> dos mais recentes <span class="elsevierStyleItalic">smartphones</span>&#41; veio permitir o registo de epis&#243;dios de FA parox&#237;stica num crescente n&#250;mero de doentes&#46;</p><p id="par0070" class="elsevierStylePara elsevierViewall">Alguns destes sistemas de an&#225;lise de tiras de ECG foram j&#225; rigorosamente testados<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> o que levou &#224; sua aprova&#231;&#227;o pelas entidades regulamentares apropriadas &#40;p&#46; ex&#46;&#58; do sistema AliveCor<span class="elsevierStyleSup">&#174;</span> que foi j&#225; aprovado pela <span class="elsevierStyleItalic">Food and Drug Administration</span> nos EUA&#41;&#46;</p><p id="par0075" class="elsevierStylePara elsevierViewall">Sendo expect&#225;vel que v&#225;rios outros sistemas semelhantes venham a ser aprovados num futuro pr&#243;ximo&#44; os servi&#231;os de cardiologia ter&#227;o de se adaptar a esta nova realidade&#44; uma vez que o afluxo de dados a analisar ser&#225; substancialmente maior&#46; Este facto levar&#225; a que um maior n&#250;mero de doentes possa ser precocemente diagnosticado e tratado com f&#225;rmacos anticoagulantes&#44;
