pacing may worsen left ventricular cardiomyopathy in patients with reduced left ventricular ejection fraction &#40;LVEF&#41; and advanced atrioventricular block&#46;</p><p id="par0065" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Objective&#58;</span> The objectives of this study were to calculate incidence and identify predictors of RV pacing&#8208;induced cardiomyopathy &#40;PICM&#41; in complete heart block &#40;CHB&#41; with preserved LVEF and to describe outcomes of subsequent cardiac resynchronization therapy &#40;CRT&#41; upgrade&#46;</p><p id="par0070" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Methods&#58;</span> An analysis of consecutive patients receiving permanent pacemaker &#40;PPM&#41; from 2000 to 2014 for CHB with LVEF &#62;50&#37; was performed&#46; PICM was defined as CRT upgrade or post&#8208;PPM LVEF &#8804;40&#37;&#46; PICM association was determined via multivariable regression analysis&#46; CRT response was defined by LVEF increase &#8805;10&#37; or left ventricular end&#8208;systolic volume decrease &#8805;15&#37;&#46;</p><p id="par0075" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Results&#58;</span> Of the 823 study patients&#44; 101 &#40;12&#46;3&#37;&#41; developed PICM over the mean follow&#8208;up of 4&#46;3<span class="elsevierStyleHsp" style=""></span>&#177;<span class="elsevierStyleHsp" style=""></span>3&#46;9 years&#44; with post&#8208;PPM LVEF being 33&#46;7&#37;<span class="elsevierStyleHsp" style=""></span>&#177;<span class="elsevierStyleHsp" style=""></span>7&#46;4&#37; in patients with PICM vs 57&#46;6&#37;<span class="elsevierStyleHsp" style=""></span>&#177;<span class="elsevierStyleHsp" style=""></span>6&#46;1&#37; in patients without PICM &#40;P<span class="elsevierStyleHsp" style=""></span>&#60;<span class="elsevierStyleHsp" style=""></span>&#46;001&#41;&#46; In multivariable analysis&#44; lower pre&#8208;PPM LVEF &#40;hazard ratio &#91;HR&#93; and as a categorical &#40;&#60;20&#37; or &#8805;20&#37; RV pacing&#41; &#40;HR 6&#46;76&#59; 95&#37; CI 2&#46;08&#8211;22&#46;0&#59; P<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>&#46;002&#41; variable were independently associated with PICM&#46; Only 29 patients with PICM &#40;28&#46;7&#37;&#41; received CRT upgrade despite an 84&#37; responder rate &#40;LVEF increase 18&#46;5&#37;<span class="elsevierStyleHsp" style=""></span>&#177;<span class="elsevierStyleHsp" style=""></span>8&#46;1&#37; and left ventricular end&#8208;systolic volume decrease 45&#46;1&#37;<span class="elsevierStyleHsp" style=""></span>&#177;<span class="elsevierStyleHsp" style=""></span>15&#46;0&#37; in responders&#41;&#46; CRT upgrade was associated with greater post&#8208;PPM LVEF decrease&#44; lower post&#8208;PPM LVEF&#44; and post&#8208;PPM LVEF &#8804;35&#37; &#40;P<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>&#46;006&#44; P<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>&#46;004&#44; and P<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>&#46;004&#44; respectively&#41;&#46;</p><p id="par0080" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Conclusion&#58;</span> PICM is not uncommon in patients receiving PPM for CHB with preserved LVEF and is strongly associated with RV pacing burden &#62;20&#37;&#46; CRT response rate is high in PICM&#44; but is perhaps underutilized&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0010">Coment&#225;rio</span><p id="par0085" class="elsevierStylePara elsevierViewall">Desde h&#225; v&#225;rios anos que est&#225; estabelecido que o <span class="elsevierStyleItalic">pacing</span> apical direito agrava a fun&#231;&#227;o ventricular esquerda&#44; em doentes com disfun&#231;&#227;o de base&#46; Por esse motivo&#44; as diretrizes atuais da Sociedade Europeia de Cardiologia recomendam que&#44; em doentes com disfun&#231;&#227;o ventricular esquerda&#44; se houver necessidade de <span class="elsevierStyleItalic">pacing</span> por bradicardia deve ser implantado um dispositivo de ressincroniza&#231;&#227;o&#46;</p><p id="par0090" class="elsevierStylePara elsevierViewall">Alguns doentes submetidos a implanta&#231;&#227;o de <span class="elsevierStyleItalic">pacemaker</span>&#44; mesmo com fun&#231;&#227;o ventricular basal normal&#44; desenvolvem no seguimento insufici&#234;ncia card&#237;aca e disfun&#231;&#227;o ventricular esquerda&#44; sem outra causa aparente que n&#227;o seja a elevada percentagem de estimula&#231;&#227;o card&#237;aca artificial &#8211; esta entidade foi denominada de miocardiopatia induzida pelo <span class="elsevierStyleItalic">pacing</span> &#40;MIP&#41;&#46; N&#227;o &#233; conhecida qual a sua real incid&#234;ncia&#44; bem como os fatores de risco para a sua ocorr&#234;ncia&#46;</p><p id="par0095" class="elsevierStylePara elsevierViewall">O presente estudo pretendeu avaliar a incid&#234;ncia de MIP numa popula&#231;&#227;o de doentes com fun&#231;&#227;o ventricular esquerda normal em quem foi implantado <span class="elsevierStyleItalic">pacemaker</span>&#46; Trata&#8208;se de um estudo retrospetivo&#44; de centro &#250;nico &#40;<span class="elsevierStyleItalic">Cleveland Clinic</span>&#41;&#44; de doentes que implantaram <span class="elsevierStyleItalic">pacemaker</span> &#40;de novo&#41; por bloqueio auriculoventricular completo entre 2000&#8208;2014&#44; com fra&#231;&#227;o de eje&#231;&#227;o &#40;FE&#41;<span class="elsevierStyleHsp" style=""></span>&#62;<span class="elsevierStyleHsp" style=""></span>50&#37; &#40;avaliada por ecocardiograma at&#233; seis meses antes da implanta&#231;&#227;o&#41;&#46; Foram exclu&#237;dos doentes em que n&#227;o existia quantifica&#231;&#227;o da fun&#231;&#227;o ventricular esquerda&#44; doentes sem dados de seguimento e doentes que implantaram dispositivos de ressincroniza&#231;&#227;o&#46;</p><p id="par0100" class="elsevierStylePara elsevierViewall">De 1147 doentes identificados&#44; 323 &#40;28&#44;2&#37;&#41; foram exclu&#237;dos &#40;por n&#227;o haver dados de seguimento ou por n&#227;o terem ecocardiograma pr&#233;&#8208;implanta&#231;&#227;o&#41;&#46; A popula&#231;&#227;o do estudo consistiu assim em 823 doentes&#46;</p><p id="par0105" class="elsevierStylePara elsevierViewall">Do total da popula&#231;&#227;o&#44; 12&#44;3&#37; vieram a desenvolver MIP &#40;definida para este estudo como FE abaixo de 40&#37; ou necessidade de <span class="elsevierStyleItalic">upgrade</span> para dispositivo de ressincroniza&#231;&#227;o&#41;&#44; num seguimento m&#233;dio de 4&#44;3<span class="elsevierStyleHsp" style=""></span>&#177;<span class="elsevierStyleHsp" style=""></span>3&#44;9 anos&#46;</p><p id="par0110" class="elsevierStylePara elsevierViewall">A idade dos doentes&#44; o sexo&#44; os antecedentes de doen&#231;a coron&#225;ria&#44; arritmias auriculares&#44; HTA&#44; diabetes&#44; IRC ou a fra&#231;&#227;o de eje&#231;&#227;o pr&#233;&#8208;procedimento n&#227;o eram diferentes no grupo que n&#227;o desenvolveu miocardiopatia quando comparado com os restantes&#46; De referir ainda que tamb&#233;m n&#227;o houve diferen&#231;as significativas quanto ao local de <span class="elsevierStyleItalic">pacing</span> no VD &#40;apical <span class="elsevierStyleItalic">versus</span> n&#227;o apical&#41;&#46;</p><p id="par0115" class="elsevierStylePara elsevierViewall">A carga de <span class="elsevierStyleItalic">pacing</span> ventricular foi diferente nos dois grupos&#58; os doentes que desenvolveram miocardiopatia tinham percentagens de <span class="elsevierStyleItalic">pacing</span> superiores&#46;</p><p id="par0120" class="elsevierStylePara elsevierViewall">Num modelo de an&#225;lise multivariada&#44; uma carga de <span class="elsevierStyleItalic">pacing</span> superior a 20&#37;&#44; assim como uma fra&#231;&#227;o de eje&#231;&#227;o ventricular mais baixa &#40;mas acima de 50&#37;&#41; na data do implante foram preditores da ocorr&#234;ncia de MIP &#40;as vari&#225;veis inclu&#237;am ainda a idade no implante&#44; in complete heart block &#40;CHB&#41; with preserved LVEF and to describe outcomes of subsequent cardiac resynchronization therapy &#40;CRT&#41; upgrade&#46;</p><p id="par0070" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Methods&#58;</span> An analysis of consecutive patients receiving permanent pacemaker &#40;PPM&#41; from 2000 to 2014 for CHB with LVEF &#62;50&#37; was performed&#46; PICM was defined as CRT upgrade or post&#8208;PPM LVEF &#8804;40&#37;&#46; PICM association was determined via multivariable regression analysis&#46; CRT response was defined by LVEF increase &#8805;10&#37; or left ventricular end&#8208;systolic volume decrease &#8805;15&#37;&#46;</p><p id="par0075" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Results&#58;</span> Of the 823 study patients&#44; respectively&#41;&#46;</p><p id="par0080" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Conclusion&#58;</span> PICM is not uncommon in patients receiving PPM for CHB with preserved LVEF and is strongly associated with RV pacing burden &#62;20&#37;&#46; CRT response rate is high in PICM&#44; but is perhaps underutilized&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0010">Coment&#225;rio</span><p id="par0085" class="elsevierStylePara elsevierViewall">Desde h&#225; v&#225;rios anos que est&#225; estabelecido que o <span class="elsevierStyleItalic">pacing</span> apical direito agrava a fun&#231;&#227;o ventricular esquerda&#44; em doentes com disfun&#231;&#227;o de base&#46; Por esse motivo&#44; as diretrizes atuais da Sociedade Europeia de Cardiologia recomendam que&#44; em doentes com disfun&#231;&#227;o ventricular esquerda&#44; se houver necessidade de <span class="elsevierStyleItalic">pacing</span> por bradicardia deve ser implantado um dispositivo de ressincroniza&#231;&#227;o&#46;</p><p id="par0090" class="elsevierStylePara elsevierViewall">Alguns doentes submetidos a implanta&#231;&#227;o de <span class="elsevierStyleItalic">pacemaker</span>&#44; mesmo com fun&#231;&#227;o ventricular basal normal&#44; desenvolvem no seguimento insufici&#234;ncia card&#237;aca e disfun&#231;&#227;o ventricular esquerda&#44; sem outra causa aparente que n&#227;o seja a elevada percentagem de estimula&#231;&#227;o card&#237;aca artificial &#8211; esta entidade foi denominada de miocardiopatia induzida pelo <span class="elsevierStyleItalic">pacing</span> &#40;MIP&#41;&#46; N&#227;o &#233; conhecida qual a sua real incid&#234;ncia&#44; bem como os fatores de risco para a sua ocorr&#234;ncia&#46;</p><p id="par0095" class="elsevierStylePara elsevierViewall">O presente estudo pretendeu avaliar a incid&#234;ncia de MIP numa popula&#231;&#227;o de doentes com fun&#231;&#227;o ventricular esquerda normal em quem foi implantado <span class="elsevierStyleItalic">pacemaker</span>&#46; Trata&#8208;se de um estudo retrospetivo&#44; de centro &#250;nico &#40;<span class="elsevierStyleItalic">Cleveland Clinic</span>&#41;&#44; de doentes que implantaram <span class="elsevierStyleItalic">pacemaker</span> &#40;de novo&#41; por bloqueio auriculoventricular completo entre 2000&#8208;2014&#44; com fra&#231;&#227;o de eje&#231;&#227;o &#40;FE&#41;<span class="elsevierStyleHsp" style=""></span>&#62;<span class="elsevierStyleHsp" style=""></span>50&#37; &#40;avaliada por ecocardiograma at&#233; seis meses antes da implanta&#231;&#227;o&#41;&#46; Foram exclu&#237;dos doentes em que n&#227;o existia quantifica&#231;&#227;o da fun&#231;&#227;o ventricular esquerda&#44; doentes sem dados de seguimento e doentes que implantaram dispositivos de ressincroniza&#231;&#227;o&#46;</p><p id="par0100" class="elsevierStylePara elsevierViewall">De 1147 doentes identificados&#44; 323 &#40;28&#44;2&#37;&#41; foram exclu&#237;dos &#40;por n&#227;o haver dados de seguimento ou por n&#227;o terem ecocardiograma pr&#233;&#8208;implanta&#231;&#227;o&#41;&#46; A popula&#231;&#227;o do estudo consistiu assim em 823 doentes&#46;</p><p id="par0105" class="elsevierStylePara elsevierViewall">Do total da popula&#231;&#227;o&#44; 12&#44;3&#37; vieram a desenvolver MIP &#40;definida para este estudo como FE abaixo de 40&#37; ou necessidade de <span class="elsevierStyleItalic">upgrade</span> para dispositivo de ressincroniza&#231;&#227;o&#41;&#44; num seguimento m&#233;dio de 4&#44;3<span class="elsevierStyleHsp" style=""></span>&#177;<span class="elsevierStyleHsp" style=""></span>3&#44;9 anos&#46;</p><p id="par0110" class="elsevierStylePara elsevierViewall">A idade dos doentes&#44; o sexo&#44; os antecedentes de doen&#231;a coron&#225;ria&#44; arritmias auriculares&#44; HTA&#44; diabetes&#44; IRC ou a fra&#231;&#227;o de eje&#231;&#227;o pr&#233;&#8208;procedimento n&#227;o eram diferentes no grupo que n&#227;o desenvolveu miocardiopatia quando comparado com os restantes&#46; De referir ainda que tamb&#233;m n&#227;o houve diferen&#231;as significativas quanto ao local de <span class="elsevierStyleItalic">pacing</span> no VD &#40;apical <span class="elsevierStyleItalic">versus</span> n&#227;o apical&#41;&#46;</p><p id="par0115" class="elsevierStylePara elsevierViewall">A carga de <span class="elsevierStyleItalic">pacing</span> ventricular foi diferente nos dois grupos&#58; os doentes que desenvolveram miocardiopatia tinham percentagens de <span class="elsevierStyleItalic">pacing</span> superiores&#46;</p><p id="par0120" class="elsevierStylePara elsevierViewall">Num modelo de an&#225;lise multivariada&#44; uma carga de <span class="elsevierStyleItalic">pacing</span> superior a 20&#37;&#44; assim como uma fra&#231;&#227;o de eje&#231;&#227;o ventricular mais baixa &#40;mas acima de 50&#37;&#41; na data do implante foram preditores da ocorr&#234;ncia de MIP &#40;as vari&#225;veis inclu&#237;am ainda a idade no implante&#44; o sexo&#44; a presen&#231;a de arritmias auriculares&#44; a localiza&#231;&#227;o do el&#233;trodo de <span class="elsevierStyleItalic">pacing</span> e a dura&#231;&#227;o do QRS&#41;&#46;</p><p id="par0125" class="elsevierStylePara elsevierViewall">A carga de <span class="elsevierStyleItalic">pacing</span> de 20&#37; &#233; at&#233; inferior &#224; descrita noutros estudos de MIP &#40;habitualmente &#233; referido o valor de 40&#37;&#41;&#46; As curvas de sobreviv&#234;ncia para os dois grupos de cargas de <span class="elsevierStyleItalic">pacing</span> &#40;&#62;<span class="elsevierStyleHsp" style=""></span>20&#37; e &#60;<span class="elsevierStyleHsp" style=""></span>20&#37;&#41; come&#231;am a divergir a partir dos dois anos de seguimento&#46;</p><p id="par0130" class="elsevierStylePara elsevierViewall">A percentagem de doentes com MIP que foi submetida a <span class="elsevierStyleItalic">upgrade</span> para CRT foi baixa &#40;30&#37;&#41;&#46; A taxa de resposta &#224; ressincroniza&#231;&#227;o foi elevada &#40;84&#37;&#41;&#44; de alguma forma comprovando que o <span class="elsevierStyleItalic">pacing</span> estava na g&#233;nese do agravamento da fun&#231;&#227;o ventricular esquerda&#46;</p><p id="par0135" class="elsevierStylePara elsevierViewall">Em conclus&#227;o&#44; a incid&#234;ncia de MIP &#233; elevada &#40;12&#37; nesta coorte de doentes&#41;&#46; O fator de risco principal para o agravamento da fun&#231;&#227;o ventricular esquerda &#233; uma carga de <span class="elsevierStyleItalic">pacing</span> superior a 20&#37;&#46; O tempo de seguimento tamb&#233;m &#233; importante&#44; uma vez que &#233; a partir do segundo ano que o risco &#233; maior&#46; As implica&#231;&#245;es dos resultados e conclus&#245;es deste estudo no seguimento de doentes com <span class="elsevierStyleItalic">pacemaker</span> s&#227;o&#58; &#40;1&#41; logo desde a data da implanta&#231;&#227;o&#44; tudo deve ser feito de forma a diminuir as percentagens de <span class="elsevierStyleItalic">pacing</span> ventricular &#40;por exemplo&#44; com aumento do intervalo AV ou com ativa&#231;&#227;o dos algoritmos de minimiza&#231;&#227;o de <span class="elsevierStyleItalic">pacing</span>&#41;&#59; &#40;2&#41; &#233; sensato o seguimento com ecocardiografia seriada ap&#243;s implanta&#231;&#227;o de <span class="elsevierStyleItalic">pacemaker</span>&#44; por per&#237;odos prolongados de tempo&#58; a diminui&#231;&#227;o da fun&#231;&#227;o ventricular esquerda deve levar a <span class="elsevierStyleItalic">upgrade</span> para sistema de ressincroniza&#231;&#227;o&#44;
