Nonbacterial thrombotic endocarditis: A review

Abstract

The entity of NBTE is reviewed in this article. Historic aspects, epidemiology, and pathogenesis are discussed. The clinicopathologic findings are emphasized as well as the potential for antemortem diagnosis and therapy. NBTE is diagnosed infrequently before death. Clinical suspicion is aroused in patients with an underlying process such as malignancy, DIC, or a spectrum of other diseases and evidence of pulmonary and/or systemic embolization. Systemic infaction must be excluded. Two-dimensional echocardiography can be utilized to confirm the diagnosis. Anticoagulation therapy with heparin may prevent embolization.

      The current guidelines of the American College of Chest Physicians recommend prompt initiation of anticoagulation unless contraindicated, once the diagnosis of NBTE is made, to reduce the risk of embolic events (Class 2 indication, Level of evidence: C).74 Most studies propose heparin products, whether unfractionated or subcutaneous, as the preferred anticoagulation therapy.1,18,74 The current literature suggest that vitamin K antagonists, such as warfarin may be less effective in NBTE, compared with heparin, particularly in malignancy-associated cases.1,74,75

      Importantly, since anticoagulation is associated with a risk of hemorrhagic conversion of ischemic stroke, a vascular neurologist should be consulted before starting anticoagulation in patients with cerebral ischemia [36]. Unfractionated heparin (UFH) has been shown to be effective in reducing the incidence of recurrent episodes of thromboembolism in NBTE in historical studies [1,32,53,54]. Low molecular weight heparin (LMWH) is considered to be as effective as UFH, can be used in ambulatory patients (in contrast to UFH), and has no drug-drug interactions (in contrast to oral anticoagulants).
